Shelter WA Training and Event Request Form

This form is for use by Shelter WA team members only. Please use this form to request technical, registration, marketing and logistical support for any Shelter WA training or event, including co-hosted training or events. Where possible this request form should be submitted at least 8 weeks before the proposed date of the training or event. Please complete a new form for each individual Event or Training and provide as much information as possible so the training or event can be booked, resourced and marketed effectively for you/the hosts.

Please add their full name. Please ensure your manager is aware of and has approved the event or training and it is in line with Shelter WA business strategy/Strategic Plan.
For the purpose of this request a workshop or webinar is considered training.
Who is hosting this Event or Training?(Required)

Please allow at least 6-8 weeks before any event or training when completing your request. Please also understand that this date may not be suitable from an operational perspective.

Full price per ticket is required here, excluding GST. Members have a code to claim member pricing on training and events. GST gets calculated automatically via our online booking system. Please use the Training and Workshops Costings Calculator to calculate your costs. This is accessible on the Shelter WA Intranet. Is the Event or Training sponsored or supported by anyone/a third party?(Required)

Please be aware that a recording of the training or event, if completed, could be a resource added to the Member Hub. If unsure if the training or event should be recorded please discuss with your manager.
Intended Use of the Recording:
This helps us understand where and how the webinar recording will be shared, and how the session should be recorded (e.g. layout, speaker focus, screen sharing, audio).
Level of Editing Required:

This helps us estimate the time, resources, and production level needed to prepare the recording before it is published or shared.
(e.g. captions, accessibility needs, approval process, deadlines)

For example, for a virtual event you may require assistance with chat management, setting up hardware, etc. If unsure, please liaise with the Events & Marketing Coordinator to clarify requirement or add a note below requesting them to follow up with you. If you are unsure how to do this please liaise with the Events & Marketing Coordinator to clarify requirement or add a note below requesting them to follow up with you.

A speaker is anyone presenting content.
Include full name, mobile. email and organisation if not from Shelter WA.
Marketing: Do speakers need a bio and headshot?
If required, please make sure contact details are provided correctly in the previous question so that the Events and Marketing Coordinator can follow up with the speaker/s.
Marketing: Will the speaker/s be using PowerPoint?
Marketing: Does/do the speaker/s wish to do a test run ahead of the Event or Training?
This is preferred for online/virtual events so please ensure your speaker is aware of this.
